Diocese of Orange — Minor Permission and Liability Release Form

I, the undersigned parent or legal guardian, grant permission for my child to participate in this school/parish/diocesan event. This activity will take place under the guidance and direction of school/parish/diocesan employees and/or volunteers from Immaculate Heart of Mary Catholic Church – VEYM – Đoàn Khiết Tâm.

As parent/legal guardian, I remain legally responsible for any personal actions taken by the above named minor participant.

I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend Immaculate Heart of Mary Catholic Church – VEYM – Đoàn Khiết Tâm, its officers, directors, employees and agents, and the Diocese of Orange, its employees and agents, chaperones, or representatives associated with the event, from any claim arising from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate the parish/school, its officers, directors and agents, and the Diocese of Orange, its employees and agents and chaperones, or representative associated with the event for reasonable attorney fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of the parish/school or the Diocese of Orange.

I authorize the making of photographs, motion pictures, video tapes, recordings or other memorializing of said event and my child participation therein, and the publication and duplication or other use thereof. I waive any rights to compensation or any right that I otherwise might have to limit or control such making or use.

I give permission to the physician, nurse, dentist or licensed care staff selected by the supervisory personnel then present to render medical, dental or other appropriate treatment deemed necessary and appropriate by the physician, nurse, dentist or licensed care staff.

VEYM Participant Agreement Form — Waiver and Release

I, the undersigned parent or legal guardian, am an adult (age of majority per State, e.g. 18 years old in California) and I am the named participant, or I am the parent/guardian of the minor who will be participating in the above-mentioned event (The Event) organized and/or sponsored by the Vietnamese Eucharistic Youth Movement in the U.S.A. (VEYM). I am fully aware that my or my child participation in The Event is totally voluntary. Meanwhile, I or my child shall comply with all applicable Codes of Conduct, and generally conduct ourselves at all times in keeping with the highest moral and ethical standards, and abide by all applicable rules of law, so as to reflect positively on ourselves, the Event, and Catholic teachings. If I or my child violate these obligations which result in bodily injury or property damage during the Event, I or my child who violated these obligations will solely pay to restore or replace any property damaged, pay any damages caused by bodily injury to an individual, and defend, protect and hold VEYM, its executive members, youth leaders, and volunteers, the local diocese, priests or other religious or clergy members, harmless, from such claims.

I am aware that The Event may involve the following activities but not limited to: running, jumping, sharing personal stories, singing, clapping, shouting, sitting for prolonged periods of time, early wake-up, sleeping in cabins, sleeping in tents, use of low-light restrooms, outdoor activities in dirt, uneven, dusty and rocky terrain, sleeping outdoors, aquatic activities, and supervised online group activities. All activities will be monitored by at least 2 adults.

I, the undersigned parent or legal guardian, hereby release, acquit and forever discharge VEYM and their employees, volunteers, agents, servants, officers, trustees, representatives, affiliates, and sponsors, in their official and individual capacities, as well as my Parish and my Diocese, from any and all liability whatsoever for any and all damages, injuries (including death) to persons, loss to property, or both, which arise during, out of, or in connection with my or my child participation in The Event, including related activities and travel to and from The Event.

I agree to indemnify, defend and hold harmless VEYM and their employees, volunteers, agents, officers, trustees, representatives, affiliates, and sponsors, as well as my Parish and my Diocese, against all claims, including claims of negligence, unintentional acts, and acts of omission, and from any and all liability, loss or damage they sustain as a result of any claims which result from or arise out of my or my child participation in The Event.

I hereby acknowledge and accept that there are certain inherent dangers and foreseeable and unforeseeable risks of harm arising from The Event activities, including sustaining bodily or emotional injury. I have knowingly and voluntarily decided to assume the risks of these inherent dangers.

I acknowledge that communicable diseases may be carried by persons on campus and that participation may put me or my child at risk of exposure; that weather conditions and Acts of God may alter plans and activities; that personal property is at my own risk of theft, damage, or loss; and that VEYM reserves the right to decline, accept, or remove me or my child from The Event at any time, with no refund for any unused portion.

I understand that VEYM, in its sole discretion, reserves the right to cancel The Event or any aspect thereof, and that I or my child may not be reimbursed for any unused portion.

I represent and warrant that I or my child is covered by comprehensive health and accident insurance. Even if not covered, I agree to complete the Health Information section and hereby release and discharge VEYM of all responsibility for any injuries, illnesses, or medical expenses incurred, and authorize VEYM to obtain necessary medical treatment and to release medical information to any medical provider to whom VEYM refers me or my child.

I agree that this agreement is construed under the laws of the State of California, is intended to be as broad and inclusive as permitted by law, and that any legal action arising out of it must be brought in Orange County, California. To the extent not prohibited by law, this release is also for negligence.

I hereby grant VEYM my consent, without reservation, to use, reproduce, publish or sell my or my child name, voice, image, and/or likeness arising from participation in The Event for promotional, instructional, business or other lawful purposes, at VEYM sole discretion.

IN SIGNING THIS AGREEMENT, I ACKNOWLEDGE AND REPRESENT THAT I HAVE READ THIS ENTIRE DOCUMENT, THAT I UNDERSTAND ITS TERMS, THAT I UNDERSTAND IT AFFECTS MY LEGAL RIGHTS, THAT IT IS A BINDING AGREEMENT, AND THAT I HAVE SIGNED IT KNOWINGLY AND VOLUNTARILY OF MY OWN FREE WILL. THIS RELEASE CANNOT BE MODIFIED ORALLY.

Diocese of Orange — Adult Hold-Harmless Waiver & Release Form

I wish to participate in the activity described above, and as a condition of my being allowed to do so, I hereby agree to defend, protect, indemnify, and hold harmless the above named diocesan parish/school against and from all claims arising from my negligence or my fault which arise out of the above named activity at the above named location.

Additionally, I agree to protect, defend, hold harmless and fully indemnify the above named diocesan parish/school for any claim or cause of action whatsoever arising out of the above named activity that is brought against the parish/school by me or my family members, whether such a claim arises from the alleged negligence of the parish/school, its employees or agents, or my own negligence.

I hereby warrant and represent that I am physically fit and capable of taking part in this activity.

I agree to abide by the rules and regulations governing the above described activity, and to obey any instructions given by the person or persons having supervision and control over the activity.

I hereby authorize the making of photographs, motion pictures, video tapes, recordings, or other memorializing of said event and my participation therein, and the publication or other use thereof. I hereby waive any right to compensation therefore or any right that I otherwise might have to limit or control such making or use.

I warrant and represent that I am eighteen years of age, or over. If any portion of this agreement is held invalid, it is agreed that the balance thereof shall continue in full legal force and effect.

Đoàn Khiết Tâm — Adult Event Agreement

I voluntarily agree to participate in this event as an adult attendee, Huynh Trưởng, or parent chaperone. I understand and accept all event-specific rules, logistics, and responsibilities as described in the event details above.

Check-In & Wristbands. Everyone — including youth participants, parents, and visitors — must check in and receive a wristband upon arrival. Wristbands must be worn and visible at all times for the duration of the event. Anyone without a wristband may be asked to check in or leave the premises. Unannounced or unregistered visitors may be turned away at the discretion of event leadership. Visitor access and hours are at the discretion of the event's leadership team and will be communicated in advance when applicable.

Youth Participant & Adult Interactions. Youth participants must remain in their designated areas at all times. Parents and non-staff adults are not permitted inside youth-only designated areas. Parents and visitors should refrain from attending exclusively to their own child(ren); if a youth participant has a need, they should notify a Huynh Trưởng, who will assist them accordingly. If a youth participant needs to be removed from an activity or from the event, the parent or guardian must first notify a Huynh Trưởng in charge before doing so.

Parking & Drop-Off. All vehicles must park in the designated parking area. Do not move barricades, cones, or signs for any reason. Follow all directions from on-site security at all times. If you need assistance unloading supplies, park in the designated area and contact a Huynh Trưởng. For overnight events, sleeping in your vehicle at any time is not permitted.

Prohibited Items & Behaviors. The following are strictly prohibited on all event grounds: alcoholic beverages of any kind; tobacco products, smoking materials, or vapor/e-cigarette devices; illegal substances or weapons of any kind; and any item deemed inappropriate or unsafe by event leadership.

General Conduct & Environment. Please clean up after yourself and others proactively. The youth participants' well-being and experience is the top priority of every event. Treat all youth participants, parents, and Huynh Trưởng with respect. Disruptive, disrespectful, or unsafe behavior will not be tolerated.

Consequences for Violations. Violation of any rule outlined in this agreement may result in being asked to leave the event immediately at the discretion of Đoàn Khiết Tâm leadership. Serious or repeated violations may result in exclusion from future events.

By signing below, I confirm that I have read, understood, and agree to abide by all terms in this agreement for the full duration of the event.
